What is a Care Coordinatos?

A Care Coordinator is your helper. They work for the NHS in Liverpool. Their job is to make sure you get the right help. They connect you to different services. This makes your health care easy to use.

What they do for you:

• They help you find support.

• They make sure your doctors work together.

• They help plan your health care.

• They can help with info.

• They can help book appointments.

• They can help your family too.

Our Health Inequalities Lead: Fair Health for Everyone

We also have a Health Lead. Their job is to make health care fair. Everyone in our area should get good care. Some people find it hard to get health help. This Lead works to fix that.

What they do for you:

• They look for problems.

• They find answers.

• They make sure help gets to everyone, so that everyone in Anfield and Everton can get good health care.
