Our Mission & Team

What is Anfield and Everton Primary Care Network?

Anfield and Everton PCN is a group of five GP practices working to support the health needs of the local community.

Our network includes:

Great Homer Street Medical Centre
Islington House Medical Centre
Mere Lane Group Practice
Priory Medical Centre
Townsend Medical Centre

Our network team includes the following roles:

Dieticians
First Contact Physiotherapists
Mental Health Practitioners
Occupational Therapists
Care Coordinators
Paramedics
Pharmacists
Social Prescribers

They work in and around the five practices to give patients easier access to more services.

Our Values
